What is Transition?
Transition means change. We all experience change at different times during our lives. This transition is about the change from being a teenager to becoming an adult. During this period, all young people experience many changes and begin to do many new things for the first time. Young people gain new rights and responsibilities.
This time of transition can be an exciting time with lots of new opportunities.
It can also be a worrying time when young people move on from familiar people and places into the ‘unknown'.
All young people should start thinking about their futures around the time they reach 14 years. The choices they make from this time onward can make a big difference to their future adult life.
The options may be more complicated for your son or daughter because of their particular disabilities and the challenges they face in life. For this reason, this transition process has been identified as critical to their future life prospects and various services have come together to make it as smooth as possible.
