Transition Timeline Mainstream Schools

The table below sets out what young people and their parents /carers can expect with regard to the support available for transition planning for statemented students attending a mainsteam school.   

Year 9 (13-14 yrs)

Y-gen

  • Meet young person in class - either observation or group work session
  • Meet young person on 1:1 basis at school
  • Attend annual review meeting at school
  • Start Learning Disability Assessment process
  • Meet  parents / carers at annual review and provide information regarding possible options for transition after Year 11/12/13/14

 

Young Person and Parents/Carers

  • Attend annual review at school
  • Review options suggested by Y-gen
  • Start to discuss possible transition options at home, as appropriate
  • Start to think about what provision they may wish to consider

 

Year 10 (14-15 yrs)

Y-gen

  • Meet young person for 1:1 interview to discuss possible options and plans and to complete draft Learning Disability Assessment plan
  • Discuss options with parent and carers
  • Assist with any further information
  • Consider future funding of support needs including transport

 

Young Person and Parents/Carers

  • Attend annual review meeting at school
  • Review possible transition options
  • Continue to discuss possible transition plans at home, as appropriate
  • Short list possible options and places to visit (e.g. colleges, training providers)

 

Year 11 (15-16yrs)

Y-gen

  • Meet young person on 1:1 basis at school and update the LDA plan if necessary
  • Attend annual review meeting at school
  • Provide  young person and parents / carers with information about possible options
  • If young person is considering leaving school, send copy of completed Learning Disability Assessment plan to him/her so that it can be sent to college or training provider with the application form
  • Collate information for funding applications if required

 

Young Person and Parents/Carers

  • Attend annual review meeting at school
  • Review options suggested by Y-gen
  • If considering remaining at school in 6th form, check with school that place on an appropriate course is available.  If so, apply for 6th form course.
  • If considering local colleges or training provision, attend Open Days/Evenings (usually held in Autumn or Spring terms)
  • Apply to colleges/training providers as soon as application forms available  (either Autumn or Spring term)
  • Send copy of Learning Disability Assessment Plan to the college/s or training providers with application forms

Year 12 (16-17yrs)

Y-gen

  • Meet young person on 1:1 basis at school
  • Complete or update draft Learning Disability Assessment plan (if young person is considering leaving school after Year 12)
  • Attend annual review meeting at school if student is leaving
  • Provide  young person and parents / carers with information about possible options
  • If young person is considering leaving school after this year,  send copy of completed Learning Disability Assessment plan to him/her so that it can be sent to college or training provider with the application form
  • Collate information for funding applications if required

  • If young person is leaving school after this year,  send copy of completed Disability Assessment plan to him/her so that it can be sent to college or training provider with the application forms
