The Transition Pathway

They are useful as they show clearly what the steps are and who has responsibility. They make what is a complicated process look manageable and comprehensive.' At the core of the transition pathway are the legal duties of each statutory service. The most effective pathways have input from disabled young people and their families and from a range of other agencies likely to be providing support or services during the transition process.
