 Barnet Learning Difficulties and Disabilities Transition Planning Service

We seek a qualified and experienced senior practitioner to provide professional supervision to a team of advisers, supporting their work and ensuring that it is delivered effectively. You will coordinate the team's work in completing S139a learning difficulty assessments to a high standard, supporting young people in out of borough placements and prioritising support to Year 9 students and prospective school leavers. You will also manage a personal caseload of young people.

Able to work independently, you'll exploit your refined interpersonal skills to build relationships with young people, their families and partner agencies. Drawing on your experience of completing assessments, you will monitor the quality of the team's written work and use our information systems to keep track of progress.  Your skills in multi-agency working will enable our partnership work to grow and develop.
