Y-gen is an information, advice and guidance service for young people living or being educated in Barnet. We work with year 9 students and upwards who have statements of special educational needs in maintained schools. We support young people up to 25 years of age.

What is transition?
Transition is the period of time where a young person has to prepare for change and make various important decisions about his/her future. These could be decisions about:

Transition timeline: Year 9 to Year 11
Year 9 Annual Review
- Starting the transition process
- A Y-gen PA will meet with you to do a guidance interview
- A Y-gen PA will attend your year 9 annual review
Year 10 Annual Review
- Have there been any changes?
- Have any circumstances changed?
- A Y-gen PA may attend your year 10 annual review if requested to do so
Year 11 Annual Review
- Putting it all into practice and preparing for change
- A Y-gen PA will help you to make a moving on plan
- A Y-gen PA will attend your year 11 annual review

"I first met my personal adviser in Year 9. She met me at school before my annual review and we wrote an action plan together. She then came to my annual review in Year 9 and talked to me and my parents about things I might want to do in the future. In Year 10 she came to see me again at school and we went through my action plan and added some more information. In Year 11 I met her several times throughout the year and we worked together on my moving on plan [S139 assessment]"

Q: What is my "Moving On Plan" (S139)?
A: An action plan that will be written with your Y-gen personal adviser
Q: When is it written?
A: You will start thinking about your moving on plan in Year 9 but it will be written with you in the year when you are planning to leave your school and move on. So it may be in Year 11, Year 12, Year 13 or Year 14
Q: What is it for?
A: It is to give information to the school, college or training provider you are moving on to about any issues or difficulties that you have and how you may need to be helped and supported. For example, it could tell them that you may need help with organising your work.
Q: Do I have to write it?
A: No - you do not have to do the actual writing as your Y-gen personal adviser will do this. It will be your plan, with your ideas and thoughts - and you should be happy with it.
Q: Who else helps with writing it?
A: Anyone who knows you - for example your family, teachers, learning support assistants, youth worker, social worker or anyone who can help to provide useful information.
Y-gen Personal Advisers have completed 98 S139 moving on plans during the past academic year!!
What's in store for the future?
Working with others
In the next academic year, Y-gen will be working hard to raise its profile in and around the community. We will be concentrating on putting young people at the centre of their transitions and enlisting the help of others to make sure that this happens!
Realistic transition planning
By working with other agencies, we can all work together to ensure that young people are involved in all stages of their transition enabling the best outcome for the young person, family and/or carer. For this, we will be working towards developing a more realistic, more person centred transition plan.
Development of guidance tools
Y-gen is currently creating guidance tools to run alongside this plan. In the future, we will start to see more multimedia profiling, more specialised resources and more projects which will be supporting young people in the best possible way.
